About One Good Hospice & Home Health

 

One Good Hospice & Home Health, under new management in 2024 and now associated with Fresno Yosemite Health Care, is an all encompassing hospice provider.  Through our customizable hospice experience, we help our patients make their own choices on how their hospice plan-of-care will go.  

What is Hospice Care?

Hospice care is provided to people who are coping with any serious, incurable life limiting illness.  Hospice care can be provided in a person's home, residential care homes, skilled nursing facilities, and assisted living facilities.  Hospice odes not mean that you are in the imminent stages of passing.  It may be appropriate for hospice prior to the imminent stage to focus on quality of life and comfort. Support is also provided to the patient family and caregivers.  Many patients are referred too late to benefit from all the services offered. 

Who provides Hospice care?

One Good Hospice has a team of caring professionals that will focus on relieving pain, anxiety, physical, emotional and spiritual comfort, as well as support for your family so that the patient may live as fully and comfortably as possible.  

Our Hospice Team:

Medical Director: who oversees all aspects of our Hospice program, and make home visits as needed.

Physicians: Your primary care physician may remain as your attending.

Nurses: Both RN and LVN's provide care, follow up visits, as needed visits, coordinate the plan of care, and instruct on the proper ways to care for your loved one, and how to use medications. 

Home Health aides: provide hands on personal care.

Medical Social Worker: provide support and counseling on the end-of-life issues.

Spiritual Counseling: Non-denominational clergy provide care and guidance to patients's family members and caregivers.

Volunteers: for non-medical visits, support and encouragement. 

Other Services

We can also offer short term inpatient care "Respite Care" when the family or caregivers need a rest from caring for the patient when pain and symptoms must be monitored closely, and controlled.  

We also offer Bereavement Care for family and caregivers for one year after passing to help cope with the loss of someone important to them. 

How much does Hospice Cost?

Hospice Care is covered by Medicare, Medi-Cal, and most private insurance companies.  Please don't let financial concerns keep you from calling us.  We can help you and your family review and understand your coverage.

  1. The developmentally disabled

  2. Families dealing with emotional needs

  3. Physically Handicapped patients

  4. and for Grandpa and Grandma
